Information Notice on the Processing of Personal Data (KVKK)
This Information Notice is prepared by Özel Omega Çare Evde Bakım Merkezi ("Omega Çare") as the data controller within the scope of Law No. 6698 on the Protection of Personal Data ("KVKK").
1. Data Controller
The data controller for your personal data processed through this website is Özel Omega Çare Evde Bakım Merkezi, based in Ankara, Türkiye. You can contact us at [email protected] for all requests under KVKK.
2. Data We Collect
Through the eligibility review form we may collect: your name, country, email address, phone number, the language you browse in, and the diagnosis and treatment-stage information you choose to share. Diagnosis and treatment information constitutes special category personal data (health data) under KVKK Article 6.
3. Purposes of Processing
Your data is processed for the sole purpose of evaluating whether our CAR-T cell therapy coordination pathway may be suitable for you, communicating the outcome of that review, and complying with legal obligations applicable to international health tourism (Regulation on International Health Tourism and Tourist's Health included).
4. Legal Basis
- Personal data: your explicit consent (KVKK Art. 5/1) and the establishment/exercise of a potential service relationship (KVKK Art. 5/2-c).
- Health data (special category): your explicit consent (KVKK Art. 6/2), processed under confidentiality obligations by persons authorised under law.
- Cross-border transfer: your explicit consent (KVKK Art. 9).
5. Recipients and Transfer
Your data may be shared with the accredited cell therapy centres named during your review, the licensed home healthcare network involved in follow-up, our service providers (email delivery, cloud storage) bound by confidentiality, and public authorities where legally required. Where the review requires it, your data may be transferred outside Türkiye on the basis of your explicit consent.
6. Retention
We retain your data only as long as the review process and applicable legislation require. If no service relationship is established, form data is deleted or anonymised after the applicable limitation period unless a legal obligation requires longer retention.
7. Your Rights (KVKK Art. 11)
You have the right to learn whether your data is processed, request information about it, learn its processing purpose and whether it is used appropriately, know domestic or foreign third parties to whom it is transferred, request correction of incomplete or incorrect processing, request deletion or destruction, object to unfavourable outcomes of exclusively automated analysis, and claim compensation for damages.
8. Security
We take the technical and administrative measures required by KVKK Article 12 to protect your data against unlawful access, processing and disclosure. IP addresses may be anonymised.
